How to take Orlistat

  • Orlistat capsules are taken one capsule up to three times daily if required.

Orlistat is a generic version of the commonly prescribed item Xenical and a stronger version of the popular over-the-counter weight loss medication Alli. Orlistat 120mg capsules contain the drug orlistat, which binds to dietary fat and stops up to one third of it from being absorbed. Orlistat 120mg capsules are the generic version of Xenical and are available now that the patent on Xenical has expired. When combined with a low calorie diet and exercise, Orlistat can help you to lose an extra 1lb for every 2lb you would lose by diet and exercise alone. In clinical trials, almost twice as many patients who received Xenical, achieved an average weight loss of 10 percent or more of their body weight in one year, compared to those on diet alone.

Orlistat is used to treat obesity; this is where you have a body mass index (BMI) of 30 or over. Orlistat can also be prescribed when your BMI is 28 or over and there are other risk factors present that make weight loss favourable.

Orlistat is taken up to three times daily, with or up to one hour after each main meal. If you eat a fat-free meal, you skip a meal or have a small snack instead, you do not need to take Orlistat as there will be no fat to absorb.

As Orlistat blocks the absorption of some dietary fats, it can also block the absorption of some fat-soluble vitamins. The fat-soluble vitamins are A, D, E and K. If you are concerned about missing some nutrients or you already suffer with a deficiency in one of these vitamins, we advise taking a multivitamin at bedtime to replace any vitamins lost from your diet as part of your Orlistat weight loss treatment.

When combined with diet and exercise, Orlistat is usually effective in aiding weight loss. Weight loss should not be rushed and crash diets should not be used. Weight loss should be viewed as a steady process in order to do it in a healthy way and increase the chances of the weight staying off. A realistic target is around 1lb per week though this can vary from person to person depending on the situation.

You should only continue to take Orlistat if you are losing weight and your BMI is above 28. If you take Orlistat for 3 months and you do not lose around 5% of your body weight you should stop taking it. It can be retried again after 6 months. If your BMI drops below 28, Orlistat may no longer be appropriate and you should stop treatment. If you are losing weight and your BMI is above 28, you can take Orlistat for up to 2 years.

If you do experience side effects from Orlistat they may include abdominal pains, bloating or wind, the urgent need to open the bowels and oily, fatty, or liquid stools. These side effects are most common at the start of treatment and are most likely to occur after eating a high-fat meal. By restricting your intake of dietary fat to less than 15g per main meal you will decrease the chances of these side effects.

Other less common side effects when taking Orlistat may include: 

Rectal pain/discomfort, soft stools, incontinence (stools), bloating (experienced by some people with type 2 diabetes), tooth/gum disorder, irregularity of menstrual cycle, tiredness.

To gain the maximum benefit from Orlistat 120mg capsules you should follow the nutrition program recommended to you by your doctor. As with any weight-control program, over-consumption of fat and calories may reduce any weight loss effect.

Orlistat can cause harmless changes in your bowel habits, such as fatty or oily stools, due to the elimination of undigested fat in your faeces. The possibility of this happening may increase if Orlistat is taken with a diet high in fat. In addition your daily intake of fat should be distributed evenly over three main meals because if Xenical is taken with a meal very high in fat, the possibility of gastrointestinal effects may increase.

The use of an additional contraceptive method is recommended to prevent possible failure of oral contraception that could occur in case of severe diarrhoea.

The use of orlistat may be associated with renal stones in patients suffering from chronic kidney disease. Inform your doctor whether you suffer from problems with your kidney.

Orlistat may modify the activity of certain drugs. If you are taking any of the following you should see your doctor before tkaing Orlistat:

  • Anticoagulant drugs (e.g. warfarin). Your doctor may need to monitor your blood coagulation.
  • Ciclosporin. Your doctor may need to monitor your ciclosporin blood levels more frequently than usual.
  • Iodine salts and/or levothyroxine. Cases of hypothyroidism and/or reduced control of hypothyroidism may occur.
  • Amiodarone. You may ask your doctor for advice.
  • Medicines to treat HIV.

Orlistat may unbalance an anticonvulsivant treatment, by decreasing the absorption of antiepileptic drugs, thus leading to convulsions. Please contact your doctor if you think that the frequency and/or severity of the convulsions have changed when taking Xenical together with antiepileptic drugs.

Orlistat is not recommended for people taking acarbose (an anti-diabetic drug used to treat type 2 diabetes mellitus).

Xenical 120mg capsules contain the drug orlistat. Xenical was first developed by Roche and became available in 1998. It has since been used safely for over 15 years to help people suffering with obesity to lose excess weight where dieting and exercise alone has been unsuccessful. Xenical capsules can be prescribed for patients who have a body mass index (BMI) of 30 or over, or 28 or over if there are other risk factors present such as heart disease, diabetes or smoking.

Xenical prevents around one third of the fat consumed as part of your normal diet from being absorbed. It does this by binding to dietary fat present in the gut and preventing it from passing through the gut walls into the bloodstream, instead it remains in the gut and passes through your system. Xenical capsules should be combined with a healthy, low calorie diet and exercise regime for maximum effectiveness. When used in this way it can help you to lose an extra 1-pound for every 2-pounds you lose naturally. In clinical trials, almost twice as many patients who received Xenical, achieved an average weight loss of 10 percent or more of their body weight in one year, compared to those on diet alone.

The patent on branded Xenical has now expired allowing you to get generic Orlistat 120mg capsules as well as branded Xenical. Orlistat is also available over-the-counter at a lower 60mg dose in Alli capsules.

You should take one Xenical 120mg capsule up to three times daily, with or up to one hour after each main meal. If you eat a fat-free meal, you skip a meal or have a small snack instead, you do not need to take Xenical as there will be no fat to absorb.

We advise our patients who take Xenical to also take a multivitamin with their weight loss treatment as it can also block the absorption of some fat-soluble vitamins. The fat-soluble vitamins are A, D, E and K. It is especially important if you already suffer with a deficiency in one of these vitamins. We advise taking a multivitamin at bedtime to replace any vitamins lost from your diet as a result of taking Xenical.

What is the difference between Xenical and Orlistat?

Orlistat is the drug contained in Xenical, which is now available as a generic treatment. Medically there is no difference between the treatments, they both contain orlistat 120mg and are made to the high manufacturing standards required of UK medicines.


Can I drink alcohol whilst I'm taking Orlistat?

Alcohol does not affect Orlistat however a glass of wine does contain approximately 130 calories. Its important to factor this in to your calorie controlled diet.


How much exercise should I be doing for the best results?

Studies show that moderate exercise - such as walking for 30 minutes every day - can result in improvements in overall health. You can add exercise to your activities throughout the day by adding a little extra walking to the things you already do. What's important is including activity regularly in your routine. Start gently and work your way up. That way you are more likely to lose weight slowly and keep it off.


If I miss a dose of Orlistat, do I double up at the next meal?

Only take one capsule at a time. If you miss a dose you can take a capsule up to an hour after you have eaten and it will still be effective. If it has been longer than hour, miss that dose and carry on until the next dose is due. Do not take two at a time.


How long will it take for me to see the results?

You should see results within 2 weeks of taking Orlistat. Weight loss should be gradual and Orlistat will help to lose an extra 1-2 lbs a week.


I want to follow my own weight loss diet and supplement it with Orlistat, is this ok?

If you have a weight loss regime that works for you and you want to accelerate your progress, then you can take Orlistat with it. Gradual weight loss works best with Orlistat and gives the best long-term results. Diets that work on a high fat and low carbohydrate levels can lead to bowel changes and unwanted side effects. Stick to no more than 15g of fat per meal to avoid these unwanted effects.


What’s the best way to lose weight and keep it off?

Fads and crash diets rarely work long term - its better to lose weight gradually and steadily.

  1. Commit to a calorie controlled diet
  2. Limit the fat in your diet to 15g a meal
  3. Plan your meals and start a low calorie diet for about 5 days before taking Orlistat.
  4. Take a multivitamin daily (best taken at night)
  5. Increase physical activity slowly


Is there a maximum amount of time that I can take Orlistat for?

There is no specific amount of time you should take Orlistat for, as long as you are losing weight steadily towards your target goal and your BMI is over 28 it is still fine to take. If your BMI is under 28, you are not losing weight or you are gaining wieght you should stop taking Orlistat.


Is there a specific time that I should start taking Orlistat?

Once you have decided to lose weight using a calorie-controlled diet aided by Orlistat you should start dieting for 3-5 days before introducing Orlistat. If you currently stick to a regime of diet and exercise you can begin taking Orlistat immediately!


Will I lose more weight if I take a higher dose?

No, you should never take more than 1 capsule as a single dose and no more than 3 capsules daily. Increasing your dose will not help you lose weight and quicker however it will significantly increase your side effects and can be dangerous.


I am going out for a meal and I am worried about the side effects – what should I do?

If you know you are going out for a meal that is likely to be high in fat, you can skip taking your dose of Orlistat with that meal. That will allow you to get on and enjoy a treat without having to worry about any unpleasant side effects.


Do I take Orlistat with each meal even if I am having more than 3 meals a day?

No, you should take to the maximum dose of 1 capsule up to 3 times daily. This dose should not be exceeded. If you are having more than 3 meals a day you may wish to work out the three that have the highest fat content and only take your Orlistat with those to maximise the effectiveness.


Do I take Orlistat with a fat-free meal?

There is no need to take Orlistat if you are having a fat-free meal. If there is no fat in the meal, there is nothing for the Orlistat to block absorption of, making it ineffective.


How does Orlistat help me lose weight?

Orlistat helps to block the absorption of fats consumed as part of your diet. As you are consuming less fat, to get the energy it requires (especially if you start an exercise regime!) it must start to break down its own fat stores instead. This results in you burning fat and hence losing weight.


I have heard I should take a multivitamin with Orlistat, is this true?

It is advisable to take a multivitamin whilst taking Orlistat. This is because Orlistat can block the absorption of fat-soluble vitamins (A, D, E & K) that you normally get from your diet. It is normally advised to take the multivitamin around an hour before bedtime when there should be no Orlistat in you system, giving you the best chance of getting the required vitamins.

·      Don’t eat for the sake of eating. How often do we reach for a snack absent mindedly whilst watching TV or reading? Most of the time you are not eating to satisfy hunger, it is a habit or for comfort. These are excess calories that quickly lead to weight gain and should be the first thing cut out of your new diet. Try replacing them with a cup of tea (no sugar!), usually this is just as effective.
 
·      Don’t eat in response to emotional triggers.  Learn to recognize the situations that often lead you to overeat, whether it’s stress, boredom or loneliness. All of these can be solved by going for a walk with friends, family or pets; it takes you away from the fridge, burns calories and releases feel-good endorphins.
 
·      Enjoy your food. It’s easy to sit and eat in front of the TV or whilst doing other things and have no bearing on how much food you have consumed. Instead try savouring the experience, eat healthy food that you enjoy and take your time eating it, enjoying the smells and textures. This gives your body time to process when it actually becomes full and usually results in less calories being consumed.
 
·      It’s not less food, it’s less calories! Just because you’re on a diet doesn’t mean that you can’t fill up or you will go hungry, you just need to select your food more wisely. Go for foods high in fibre, such as fruits, vegetables, beans and whole grains, they take longer to digest and as such leave you feeling fuller for longer. Fruits & vegetables also contain a high water content, helping to keep you hydrated and making them hard to overeat.
