Thinking about swapping Mounjaro for Foundayo? You're probably weighing up more than just injections versus tablets. You might be wondering whether a daily tablet would suit your lifestyle better, whether you'll maintain progress or even whether switching is worth it.

This guide explains what to expect, how the switch works and the key things to think about before making a decision.

Switching From Mounjaro to Foundayo (Orforglipron): Key Info

  • In clinical trials, people using Mounjaro achieved 22.5% average weight loss over 72 weeks. In separate clinical trials people using Foundayo lost an average of 12.4% body weight.
  • There's no like-for-like dose swap when it comes to switching because the treatments contain different active ingredients.
  • You’ll usually start Foundayo around the time your next Mounjaro dose would have been due, but your prescriber will confirm the timing.
  • Foundayo has sometimes been referred to as the “Mounjaro pill”, but this is incorrect. Both are made by the same company, Eli Lilly, however, Mounjaro’s active ingredient is tirzepatide, while Foundayo’s active ingredient is orforglipron.

Can You Switch From Mounjaro to Foundayo (Orforglipron)?

Yes, but it's worth understanding what you gain by switching and what you give up. While many people like the idea of swapping injections for a tablet, whether switching is right for you depends on your priorities, lifestyle and weight-loss goals.

Can You Switch From Wegovy Injections to Foundayo?

Yes, under medical supervision. If you’re using Wegovy injections and you’d like to switch to Foundayo (orforglipron), your prescriber will be able to guide you through the transition. This will depend on how you’ve responded to Wegovy, any side effects you’ve experienced and whether Foundayo is suitable for you.

Who Is Eligible To Switch From Mounjaro to Foundayo (orforglipron)?

You’re generally eligible to switch from Mounjaro injections to Foundayo if you:

  • Are an adult
  • Are established on treatment and haven’t had significant recent side effects, such as nausea, vomiting or diarrhoea
  • Have a BMI of 30+ (or 27+ with a weight-related condition, such as high blood pressure)
  • Are not pregnant or breastfeeding

You’ll need to wait a week from your last injection before you start taking Foundayo (orforglipron) daily.

You won’t necessarily need to meet the starting BMI criteria again when switching treatment because it may have reduced as a result of successful treatment. However, if you’re joining The Independent Pharmacy from another provider, we may ask for evidence of your previous treatment or starting weight. This could include your prescription or dispensing label, for example.

Why People May Decide To Switch to Foundayo

There are several reasons why you might decide to switch from Mounjaro injections to Foundayo tablets, including:

  • You're ready to leave injections behind – maybe you’re at a maintenance stage, you've never really got on with injections, or perhaps they're becoming more of a hassle than they used to be.
  • A daily tablet fits your routine better – some people prefer building treatment into their everyday routine rather than remembering one injection every week.
  • You want something easier to travel with – if you're often away from home, carrying a pack of tablets may feel simpler than travelling with injection pens, which require a sharps bin and refrigeration.

What Foundayo Dose Will You Switch To?

It would be nice if switching were as simple as matching one dose to another. Unfortunately, it doesn't quite work like that. That’s because Mounjaro and Foundayo contain different active ingredients and come in different formats.

Your starting Foundayo dose will depend on your current Mounjaro dose, how long you’ve been using it and how well you’ve tolerated treatment. As a guide:

Current Mounjaro DoseProposed Foundayo Starting Dose

If you’re established on a higher Mounjaro dose and tolerate it well, you may not need to restart Foundayo at its lowest dose. However, 9mg is the highest starting dose used in the switching protocol, and nobody should start directly on 14.5mg or 17.2mg. Your prescriber will review your treatment history, side effects and any treatment breaks before deciding the safest starting dose for you.

Increasing Your Dose After Switching

You’ll typically increase your dose each month until you reach a maintenance dose that works for you, which may or may not be the highest dose available.

You should also know that finding the right dose isn't a race. Increasing gradually gives your body time to adjust and helps reduce the chance of side effects.

How the Switching Process Works

You’ll usually take your final Mounjaro injection and then start your first dose of Foundayo when your next weekly treatment would have been due. So if your usual injection day is Tuesday, you’ll wait until the following Tuesday to start Foundayo.

This helps avoid the treatments overlapping and gives you a clear point at which to begin your new routine. However, it’s always best to check with your prescriber, who may recommend different timing depending on your dose, side effects and medical history.

How Will Switching From Mounjaro to Foundayo Impact My Weight Loss Progress?

If you've been getting great results with Mounjaro, it's completely understandable to ask whether switching means compromising on weight loss.

While Mounjaro has produced greater average weight loss in clinical trials, the treatments have not been directly compared, so switching is not automatically the wrong decision. A treatment that fits your lifestyle and that you can take consistently may be the better long-term choice.

Are the Side Effects of Foundayo Better or Worse Compared to Mounjaro Injections?

Hoping switching to a tablet might be easier on your stomach? There’s no evidence that Foundayo consistently causes fewer side effects than Mounjaro. Both can cause digestive side effects, such as nausea, vomiting, diarrhoea and constipation.

There is some reassuring evidence for people switching treatments. In the ATTAIN-MAINTAIN trial, only around 1 in 10 people switching from tirzepatide to orforglipron experienced nausea, vomiting or diarrhoea during the first four weeks, and most cases were mild to moderate.

How to Safely Stop Taking Mounjaro Injections

Mounjaro doesn’t usually need to be tapered gradually before you stop it; you can simply stop taking it. However, you should still agree on your stopping date and next treatment with your prescriber rather than changing your dose or schedule yourself.

If you’re switching to Foundayo, you’ll usually take your final Mounjaro injection and begin with the tablet when your next injection would have been due.

How Foundayo (orforglipron) Works

Orforglipron is a small-molecule GLP-1 medicine designed to be absorbed through the digestive system. It works like other GLP-1 medications and helps to reduce hunger and cravings.

To find out more about how Foundayo works, take a look at our Foundayo treatment page.

How To Take Foundayo (orforglipron)

Orforglipron is a small-molecule GLP-1 medicine that you take as a tablet. Once it’s absorbed, it activates GLP-1 receptors involved in appetite control.

Think of it as helping the gut and brain stay on the same page. After you eat, your gut naturally releases GLP-1, which sends signals to your brain that you’re getting full. Orforglipron copies that effect, helping you feel less hungry, fuller for longer and more in control of your cravings.

What To Expect After Switching From Mounjaro to Foundayo

Wondering if you’ll feel different when you make the switch from Mounjaro injections to Foundayo (orforglipron)? Here’s a brief overview of what you may experience over the coming months.

Short-Term (1-3 Months)

In the first 3 months, you may:

  • Need to make practical adjustments to your routine, because instead of remembering one weekly injection, you’ll need to build a tablet into your daily routine.
  • Experience appetite changes as the effects of Mounjaro wear off or while your Foundayo dose is being increased.
  • Find you have similar or new side effects, such as nausea, indigestion, bloating, diarrhoea or constipation, while your body adjusts to orforglipron, particularly when you increase your dose.
  • Either continue losing weight steadily or experience an adjustment period. This doesn’t mean it’s not working; it’s just that your body is getting used to the new treatment.

Long-Term (6+ Months)

After six months, you’ll usually have a better sense of how Foundayo fits with your routine, appetite and weight-loss goals. Depending on how you respond and any side effects you experience, you may also have reached your maintenance dose.

By this stage:

  • You may find hunger and cravings easier to manage, while feeling fuller for longer.
  • Foundayo may feel easier to fit into your routine, if you prefer a daily tablet to a weekly injection.

When To Speak With a Specialist

Switching to Foundayo (orforglipron) is a big change, and it's completely normal to have questions along the way. Whether you're deciding if it's the right option or you've already made the switch, our clinical team is here to support you.

Get in touch if:

  • You're unsure whether switching is the right choice for you.
  • You're finding it difficult to build Foundayo into your daily routine or have questions about taking it correctly.
  • Your side effects aren't improving or are making it difficult to continue treatment.
  • You're concerned about your appetite, weight loss or overall progress.
  • You'd like advice about taking the Foundayo alongside your other medicines or managing an existing health condition.

Can You Switch Back to Mounjaro Injections if Foundayo Doesn't Work for You?

Yes. If Foundayo isn't the right fit for you, you may be able to switch back to Mounjaro injections after discussing your options with our clinical team.

Reasons you might want to switch back include:

  • Difficulty remembering a daily tablet
  • Side effects that don't improve over time
  • Not achieving the weight-loss results you were hoping for
  • Preferring the convenience of a once-weekly injection

Your prescriber can advise on the safest way to transition between treatments and recommend the option that best supports your long-term weight-loss goals.

When Is Switching to Foundayo (orforglipron) Not The Right Option?

Switching to Foundayo may not be the best option for you if:

  • You're happy with your current Mounjaro routine. If you're comfortable with a weekly injection and it's fitting well into your lifestyle, you may not benefit from changing to a daily tablet with more specific administration requirements.
  • Your prescriber thinks Mounjaro remains the better option for you. Mounjaro and Foundayo contain different active ingredients. If you're responding well to Mounjaro and tolerating it, your prescriber may recommend staying on your current treatment.

Can You Switch From Other Weight Loss Injections to Foundayo?

Yes, you can switch from other weight loss injections, such as Wegovy injections, to Foundayo (orforglipron) under medical supervision.

The switch requires your pharmacist to carefully select your starting dose and plan the transition to help maintain your appetite control and minimise any side effects. They’ll recommend the most appropriate starting dose and timing based on your current treatment.

Interested in Switching?

Switching isn't about choosing the newest treatment or the one with the biggest weight-loss percentage. It's about finding the treatment that works best for you.

Whether weight loss injections or a pill is suitable for you will depend on your current treatment, how well it’s working for you and whether a daily tablet routine works for your lifestyle.

FAQs: Switching From Mounjaro Injections to Foundayo (orforglipron)

Is Foundayo (orforglipron) better than Mounjaro injections?

Not necessarily. They both help reduce appetite and support weight loss, but they won't be the right choice for everyone. Mounjaro has produced greater average weight loss in indirect clinical trials, while Foundayo offers you the convenience of a once-daily tablet. The best option for you depends on your weight loss goals, which routine you're most likely to stick with and which one suits you best.

How effective is Foundayo (orforglipron) compared to Mounjaro injections?

Both treatments can lead to significant weight loss when combined with a reduced-calorie diet and increased physical activity. Mounjaro has shown greater average weight loss in separate clinical trials. However, because the treatments weren't compared directly, these results aren't like-for-like.

Is Foundayo (orforglipron) cheaper than Mounjaro?

It may be, but cost is only one factor to consider. Prices can vary depending on the dose and where you receive your treatment. When deciding whether to switch, it's also worth thinking about convenience, your weight-loss goals and which treatment fits your lifestyle best.

Does switching my GLP-1 treatment mean I have to start from scratch?

No. Although you'll be starting a different medicine, that doesn't mean you're restarting your weight-loss journey. Your prescriber will recommend an appropriate starting dose and gradually increase it over time, while your healthy habits and the progress you've already made provide a strong foundation for what comes next.

Do weight loss injections work faster than pills?

Not necessarily. How quickly you lose weight depends on many factors, including the treatment you're taking, your dose, how consistently you use it and your lifestyle. Injectable treatments such as Mounjaro have produced greater average weight loss in some clinical trials, but these were not head-to-head comparisons. Both the injections and tablets are designed to support gradual, sustainable weight loss rather than rapid results.

How long should you wait between your last Mounjaro injection and your first Foundayo tablet?

You’ll usually wait one week. In most cases, you'll start Foundayo on the day your next Mounjaro injection was due. For example, if you normally inject on a Wednesday, you'd take your first tablet the following Wednesday.

It’s always best to discuss your individual situation with your prescriber to confirm the appropriate timing and dosage for your switch.

Will Foundayo (orforglipron) be available on the NHS?

Availability on the NHS will depend on future NICE guidance and local commissioning decisions.

Can you take Mounjaro injections and Foundayo (orforglipron) at the same time?

No. Both medicines work on similar appetite-regulating pathways and shouldn't be used together unless advised by a specialist. If you're switching treatments, your prescriber will explain exactly when you should stop taking Mounjaro and when you should start taking Foundayo.
